3.资格要求
3.1 符合《中华人民共和国政府采购法》第二十二条对供应商的资格要求；
3.2 具有独立法人资格的生产企业或经销商；
3.3具有有效的营业执照、组织机构代码证、税务登记证、开户许可证；
3.4本项目不接受联合体报名
4．报名时间及询价通知书获取方式
4.1 本项目采取网上报名。
已在滑县公共资源交易中心注册并确认过的投标单位可直接在网上报名。
未注册登记的投标单位，注册须另提交相关资料一套：
投标单位报名之前，须根据滑县公共资源交易中心网站“公告通知”栏目中《投标单位登记及信息变更操作流程》先进行网上注册，并按照通知要求提交相关证件到滑县公共资源交易中心大厅信息股进行核对（咨询电话0372—5576055）确认后即可网上报名。
注意事项：
本项目开标时，前来投标的单位被授权委托人必须通过滑县公共资源交易中心登记，否则将导致其投标无效。
4.2凡有意参加投标者，请于2015年5月8日8时00分至2015年5月13日18时00分登录“滑县公共资源交易中心网站”，凭企业用户名与密码进行网上报名，并下载询价通知书。
4.3本项目的所有变更和澄清均以发布变更或澄清公告为准，所有投标人应随时关注网站及时查看，采购人不再进行书面或其他形式通知，未及时查看变更或澄清公告并由此导致的一切后果均由投标人自负。
4.4询价通知书费：
人民币200元/单位/标包，本项目须在递交投标文件时现场缴费，否则，招标代理公司有权拒收其投标文件。
4.5 本项目还要实行资格后审，供应商须在询价会议召开时提供资格审查要求的所有相关证书原件。
开标后由评标委员会对投标单位的资格证明材料进行资格审核，不符合项目资格条件的投标单位的投标将被拒绝，投标单位应自负风险费用，提供虚假材料的将进一步追究其责任。
5. 询价响应性文件的递交
5.1询价响应性文件递交截止时间：
2015年5月15日9:00整（北京时间）。
5.2询价响应性文件递交地点：
滑县公共资源交易中心四楼第二开标室。
5.3 未按询价通知书规定逾期送达的或者未送达指定地点的供应商文件，采购人不予受理。
